Current license as a Registered Nurse, Physical Therapist, Occupational Therapist or Speech Language Pathologist in the State of Hawaii. Within two (2) months of date of hire, CPR certification (Health care provider course or equivalent which includes neonatal, child, adult and AED’s).
The REHAB Community Liaison I performs activities and duties related to potential patient identification, preadmission assessment screening, and admission of patients to services along the rehabilitation continuum of care. The primary role of the REHAB Community Liaison I is two-fold: 1) to identify individuals who meet program criteria and who will benefit from program services, and 2) to promote and market REHAB’s programs and services. The Community Liaison I works under the direction of the Director of Community Development. In the course of performing job duties, the REHAB Community Liaison I also serves as a clinical practitioner, educator, advocate, consultant, facilitator, negotiator and collaborator.
