Inpatient Admissions
AAA

REHAB accepts inpatient referrals from physicians, case managers, discharge planners, commercial carriers and other healthcare organizations utilizing general acute rehabilitation criteria. Upon receipt of a new referral, REHAB’s admissions office will gather specific information about the patient’s illness or injury, including insurance provider. Once admission is processed, preparations for inpatient care can be made.

Transportation to REHAB

Referring hospital will inform patients about the available options and costs when arranging transportation to REHAB. If patients are transported by family or friends, it is recommended that central registration office is contacted upon arrival for wheelchair assistance.

Hospital Entrance/Drop Off

REHAB inpatient entrance is located in the back of the building. When turning into the REHAB parking lot off of Kuakini street, drive up along the diamond head side of the hospital and turn left. Central registration office is conveniently located inside the lobby by the elevator.

What to Wear and Bring

Patients are encouraged to wear their own clothes as daily function of dressing themselves is part of the recovery process. All personal items should be labeled prior to arrival.

  • Suggested personal clothing: two weeks worth of shirts, shorts, pants and socks. Slippers and rubber-soled covered shoes are also recommended. Patients are welcomed to use hospital gowns, however, due to air conditioning, please have a light sweater or jacket available.
  • Toiletries: Toothbrush, shampoo, conditioner, body wash, mouth wash, razor, shaver, tissue, etc.
  • Insurance card(s), including prescription drug cards and a valid picture I.D. (patients may send cards home with family once copies are made).

Valuables

Patients are highly encouraged to have money and/or valuables kept at home for safekeeping. If unable to do so, REHAB’s lock boxes are available for use. Please note that REHAB cannot be responsible for lost, stolen or damaged valuables.

Admission Process

Upon arrival to REHAB, care begins with central registration. Depending on the patient’s condition, the registration process will be completed in the office or in the patient room. During the registration process, the registrar will:

  • Obtain or verify patient’s personal and insurance information
  • Make copies of insurance card(s) and picture ID for identification purposes
  • Verify transfer packet to ensure that pertinent documents from the referring hospital are received
  • Request signatures on admission and consent forms
  • Take a picture to file in patient’s chart (another form of identification for caregivers)
  • Provide wristband with patient information and corresponding REHAB physician’s name (patients are required to wear the wristband throughout their stay for safety reasons)

We welcome patient’s family and friends to participate in the healing process. Inform the nurse in charge if patient’s caregiver plans to visit outside of general visiting hours.
